All students trying out for a winter sport must have submitted the following forms to the Athletic Office by Friday, November 10th:

1) PIAA Comprehensive Pre-Participation Physical Evaluation (CIPPE)

2) IMPACT Concussion Testing Consent/Waiver Form (submit once per school-year**see note below)

3) PIAA CIPPE Section 7 form, if required, dated after October 6th, for the winter sports season. To determine whether or not you must submit Section 7, please see the informational flow chart posted on the Athletics page of the UHS website (Under Sports Physical Exam Form). In general, if you were a fall sport athlete, you must submit this form.

4) For WRESTLERS ONLY: PIAA CIPPE Section 9 (Minimum Wrestling Weight) must also be completed.
This form will be completed by the UHS Athletic Trainer and need not be submitted with a student-athletes other PIAA CIPPE forms. The mandatory testing necessary to complete the required Section 9 form will be done by the Athletic Trainer on Tuesday, November 14th, at 3:00pm. Anyone that does not complete the testing at UHS on the date offered will be ineligible to wrestle. Questions on this testing should be referred to Athletic Trainer, Joe Vogler, at .

All of these forms are available on the Athletics page of the UHS website, in the Athletic Office or Main Office, or by using the links below:

All forms must be completed and turned in PRIOR to participation in practices or tryouts.

Any athlete arriving for the first day of practices/tryouts with CIPPE and IMPACT forms will be sent to the Athletic Office to submit their forms and will not be allowed to participate until they receive written authorization.

The first day of tryouts for all UHS winter sports' teams is Friday, November 17th.

Freshman student-athletes should plan to attend tryouts beginning on this date.

**Note: Clarification on the IMPACT Concussion Testing Consent/Waiver Form Requirement:
ALL student-athletes must submit a completed IMPACT Concussion Testing Consent/Waiver Form once per school year. If you have not already done so, the IMPACT Concussion Testing Consent/Waiver form should be submitted, along with the required PIAA CIPPE (Sports Physical) forms, by November 10th. The PIAA CIPPE and IMPACT Consent/Waiver forms MUST Be submitted prior to participation
in tryouts/practices.

To clarify our requirements for IMPACT Concussion testing, we do not test all athletes each year. We administer baseline tests to our freshmen, junior and new student-athletes. We require the IMPACT Consent/Waiver form to be on file for each athlete in case post-injury testing is needed during the season. Freshmen and juniors that need to be tested will be notified by their coach and/or the Athletic Office and testing times will be scheduled early in the season. Completion of the IMPACT baseline test is NOT required prior to participation, only the submission of the IMPACT Concussion Consent/Waiver form is required before tryouts/practices.

Any questions or concerns on these topics should be directed to the UHS Athletic Office at (610) 347-1600, Ext. 3082.
