IMPORTANT 2016-17 Fall Sports Information & Deadlines

Forms & Deadlines
The first day of tryouts for UHS fall sports teams is Monday, August 15th. 
(Football begins Monday, August 8th)

Freshman student-athletes should plan to attend tryouts beginning on this date.

Practices/Tryouts for the Rowing Club will begin on Monday, August 29th.

All students trying out for a fall sport (including Rowing Club) must turn in a PIAA Comprehensive Pre-Participation Physical Evaluation (CIPPE) and a completed IMPACT Concussion Testing Consent/Waiver Form to the Athletic Office by Monday, August 8th. (Football forms are due Monday, August 1st).

These forms are available on the Athletics page of the UHS website, in the Athletic Office (Room 176) or in the Main Office. All CIPPE and IMPACT Consent/Waiver forms must be dated after May 31, 2016 for the 2016-17 school-year.

All forms must be completed and turned in PRIOR to participation in practices/tryouts. 

Any athlete arriving for the first day of practices/tryouts with CIPPE and IMPACT Consent/Waiver forms will be sent to the Athletic Office to submit their forms and will not be allowed to participate until they receive written authorization.

Parent Meeting
There will be an informational meeting held for parents of student-athletes interested in UHS fall sports. This meeting will be held in the UHS Cafeteria LGI on Monday, August 8th, at 5:00 PM. Please use the Gymnasium entrance doors. Information will be presented by Athletic Director, Joe Derickson, and should last approximately 30 minutes.

Clarification of the IMPACT Consent/Waiver Form Requirement
All student-athletes must submit a completed IMPACT Concussion Testing Waiver/Consent form once per school-year. The IMPACT Concussion Testing Consent/Waiver form should be submitted along with the required PIAA CIPPE (sports physical) forms by August 8th (Football - August 1st). The PIAA CIPPE and IMPACT Consent/Waiver forms MUST be submitted prior to participation in practices/tryouts.

To clarify our requirements for IMPACT testing, we do not test all athletes each year. We administer baseline tests to our freshmen, junior and new student-athletes. We require the consent/waiver form to be on file each year in case post-injury testing is needed during the season. Freshmen and juniors that need to be tested will be notified by their coach and/or the Athletic Office and testing times will be scheduled early in the season. Completion of the baseline test is NOT required prior to participation, only the submission of the consent/waiver form is required before tryouts/practices.
